OFFICIAL RULES (KOTV)

The following describes the Official Contest Rules for the 2025 Vision for Heroes Lasik Giveaway (the “Contest”). Contest entrants must be eligible and qualify to win by satisfying the term, requirements, and conditions set forth below:

1. NO PURCHASE NECESSARY TO WIN: A PURCHASE WILL NOT INCREASE YOUR CHANCES OF WINNING. VOID WHERE PROHIBITED BY LAW. OPEN TO LEGAL RESIDENTS OF THE STATE OF OKLAHOMA WHO ARE 18 YEARS OF AGE OR OLDER, AND LIVE WITHIN THE NEWS ON 6 DESIGNATED MARKET AREA, AS DEFINED BY NIELSEN MEDIA RESEARCH, INC. 2. SPONSOR: The sponsor of the Contest is Griffin Media (“Sponsor”), 303 N. Boston Ave., Tulsa, Oklahoma, 74103.

3. CONTEST FACILITATOR AND CONTEST OFFICIALS: The Sponsor shall also serve as the “Contest Facilitator” with responsibility for administering the Contest. “Contest Officials” designated by the management of the Contest Facilitator shall be responsible for interpretation and enforcement of the Contest Rules. Contest Officials shall have the right to disqualify any participant for violation of the Contest Rules and to resolve all disputes in their sole discretion. The Contest Facilitator reserves the right, in its sole discretion, to alter, change, cancel or suspend this Contest, for any reason whatsoever, without prior notification.

4. ELIGIBILITY: This Contest is open to all U.S. citizens and legal U.S. residents, eighteen (18) years of age as of the date they enter, who live in the State of Oklahoma, and reside within the News On 6 DMA, except as otherwise provided in these contest rules. Current employees of Sponsor and their designees or agents or their parent companies (including but not limited to Griffin Media), affiliated stations or companies (including but not limited to News 9 (KWTV), News on 6 (KOTV), The Tulsa CW (KQCW), KSBI, K-Hits 106.9, 98.5 The Bull, Big Country 99.5, and 92.9 The River), subsidiaries, divisions, and their advertising and promotional agencies and their respective officers, directors, and agents (collectively the “Contest Entities”) and the immediate family members and persons living in the same household of each, are not eligible to enter or win.

5. ELIGIBILITY RESTRICTIONS: All nominees must be either a Hero (First Responder) or an Educator certified with the State of Oklahoma, active or retired. For purposes of this giveaway, a “Hero” is defined as a police officer, firefighter, or Emergency Medical Technician (“EMT”). Winners may be required to provide proof of eligibility upon selection.

Only one (1) prize may be awarded per household for the duration of this contest. Individuals who have won a prize in any other contest hosted or conducted by GRIFFIN MEDIA must wait thirty (30) days from the date they were notified of their most recent win before becoming eligible to win again. Likewise, any winner of this contest will not be eligible to win a prize from any other contest hosted or conducted by GRIFFIN MEDIA for thirty (30) days following the date they are notified of their winning status.

6. CONTEST PERIOD: The Contest begins at 5:00 AM Central Time (“CT”) on October 10, 2025, and ends at 11:59 PM CT on October 26, 2025. The clock of the Contest Facilitator is the official Contest clock. Entries received after this deadline shall be deemed null and void and will not be included in the prize winner selection.

7. HOW TO ENTER: During the Contest Period, participants may submit a nomination form by visiting www.NewsOn6.com/Vision or www.KHITS.com/Vision. Participants may nominate themselves or someone they know.

All entries must include the following information:

• Nominator’s Name;

• Nominator’s Age;

• Nominator’s Town;

• Nominator’s Phone Number;

• Nominator’s Email Address;

• Name of Nominee (if not self-nominated);

• Nominee’s Town;

• Nominee’s Phone Number;

• Nominee’s Email Address;

• Nominee’s Qualifying Profession (as described in Section #5); and

• A brief explanation of why Nominee deserves to be recognized and considered for this prize.

If a participant does not have an email address or phone number, they should respond with “None.” However, either a phone number or email address is required for both the nominator and the nominee to be eligible in the event of selection. Entries missing this information will be deemed null and void.

Entries must be received by 11:59 PM on October 26, 2025. Only one (1) entry per person, per day, will be accepted. Photocopies, facsimiles, or other reproductions will not be eligible.

8. ALTERNATE FORM OF ENTRY: To enter using the Alternate Form of Entry, participants may submit a completed Entry Form via email at contest@NewsOn6.net with “GRIFFIN MEDIA: 2025 VISION FOR HEROES LASIK GIVEAWAY” in the subject line or by mail to Griffin Media, addressed as follows: “GRIFFIN MEDIA: 2025 VISION FOR HEROES LASIK GIVEAWAY, 303 N. Boston Ave., Tulsa, OK 74103.” All Alternate Forms of Entry must be legible and must include the same information as the online entry form, more specifically as follows:

• Nominator’s Name;

• Nominator’s Age;

• Nominator’s Town;

• Nominator’s Phone Number;

• Nominator’s Email Address;

• Name of Nominee (if not self-nominated);

• Nominee’s Town;

• Nominee’s Phone Number;

• Nominee’s Email Address;

• Nominee’s Qualifying Profession (as described in Section #5); and

• A brief explanation of why Nominee deserves to be recognized and considered for this prize.

In the event an Alternate Form of Entry is incomplete or is not legible, such entry will be deemed null and void, and will not be included in the Winner Selection process. Participant who submitted such entry may, or may not be notified if this occurs. At the end of promotional period, all valid entries received, either by online entry form or by the Alternate Form of Entry, will be combined, and one (1) winner will be selected by Contest Officials, based on responses included on the Entry Forms. All Alternate Forms of Entries must be received on or before 11:59p.m. (CST) on October 26, 2025.

9. PRIZE AND SELECTION: At the end of the promotional period, one (1) winner will be selected by contest officials from all eligible entries received. The winner will receive one (1) free LASIK Laser Vision Correction procedure from Triad Eye Institute (commonly referred to as “LASIK Eye Surgery”).

• Total Approximate Prize Value: $4,790. Any costs exceeding this amount will be the sole responsibility of the winner.

• Consultation Requirement: Triad Eye Institute will schedule a consultation with the Winner prior to the procedure to determine the Winner’s specific needs. If the total cost of the procedure exceeds $4,790, Triad will notify the winner. The winner may either cover the additional costs or forfeit the prize entirely.

• No Cash Value: The prize has no cash value. If the total procedure costs are less than $4,790, the winner will not receive the unused portion as cash. The winner will be taxed only on the actual value of the prize received.

The Prize, or any portion thereof, is non-transferable, non-refundable, non-exchangeable and has no cash value.

Prizewinner shall not be eligible to participate in or win any other contest or promotion hosted or conducted by the Contest Entities for thirty (30) days immediately following the date of winner is notified of their winning status.

10. LASIK RISK ACKNOWLEDGMENT: LASIK eye surgery carries risks, including vision complications or permanent loss. By accepting this prize, the winner acknowledges and assumes all risks associated with the procedure.

11. PRIZE DISTRIBUTION, CONDITIONS, AND ODDS OF WINNING:

Winner will be required to fill out a W9 and return it to the Contest Facilitator before the prize is awarded. Winner has thirty (30) days from the time of being notified of their winning status to return their completed W9 to the Contest Facilitator. Failure to comply could result in the winner being disqualified and their prize revoked by the Contest Facilitator, with or without notice.

Winner must have a valid ID to claim their prize. Contest Facilitator reserves the right to require additional identification and may choose to deny awarding the prize based on falsification of identification, fraud, ineligibility, or other failure to comply with the Contest Rules.

All prizes may be revoked by the Contest Administrator under the additional following conditions: (1) a winner cannot be reached within twenty-four (24) hours of winner selection; (2) Contest Facilitator is unable to leave a voicemail due to a full voicemail box or because the mailbox is not properly set-up; (3) a winner does not return a phone call within forty-eight (48) hours of a message left by Contest Facilitator; (4) a winner is determined to be ineligible for the Contest; or (5) any other reason set forth in the Contest Rules.

Odds of winning depend on the number of contest entries received.

12. TAXES AND COSTS RELATED TO PRIZES: All federal, state, or other tax liabilities (including income taxes), and subsequent or related expenses such as, but not limited to, gasoline, insurance, and maintenance arising from the award of a prize from this Contest will be the sole responsibility of each winner. In the event a prize winner receives, or has already received, a prize from any other contest(s) sponsored by Sponsor, Contest Facilitator and/or any of the Contest Entities, during the 2025 calendar year, such prize values will be combined with this prize value. Winner will receive a Form 1099 from Sponsor for total value of all prizes received during the 2025 calendar year.

13. GENERAL CONDITIONS: By participating in this Contest, all entrants agree (a) to abide by these and any additional specific official rules and the decisions of the Contest Entities and Contest Officials designated by Contest Facilitator, which will be binding and final in all respects; (b) to the use of information by the Contest Entities for marketing and advertising purposes as allowed by applicable privacy policies and law; (c) to indemnify, defend, and hold harmless the Contest Entities and their officers, directors, employees and agents from any and all liability, claims, lawsuits, costs, expenses, fees, injuries (including death), losses, and damages resulting or alleged to result from participation in the Contest, acceptance or use of any prize, and/or from entrants fraudulent or otherwise wrongful acts or omissions; (d) not to sue the Contest Entities for any damages alleged to arise from winner’s acceptance of or use of the prize(s); and (e) to the use of the entrants, name, voice, biography, and likeness by Contest Entities without further notice or compensation for promotion, advertising, filing, and dissemination purposes, except where otherwise prohibited by law.

Contest Entities are not responsible for lost, late, damaged, misdirected, undelivered, undeliverable, incomplete, or unreadable entries. Contest Entities are not responsible for lost entries due to telephonic or electronic malfunctions. Contest Officials may prohibit an entrant from participating in the Contest or winning a prize if, in their sole discretion, it determines that said entrant is attempting to undermine the legitimate operation of the Contest by cheating, deception, or other unfair playing practices or intending to annoy, abuse, threaten, or harass any other entrants or Contest Entities representatives. If for any reason this Contest is not capable of running as planned as a result of any causes beyond the reasonable control of Contest Entities including but not limited to tampering, unauthorized intervention or fraud which corrupt or affect the administration, security, fairness, integrity, or proper conduct of the Contest, then, Contest Officials reserve the right at their sole discretion to cancel, terminate, modify, or suspend the Contest. The rules can be modified at any time without notice.

14. DISPUTES: All entrants and Sponsor (together “the Parties”) agree that: (1) any and all disputes, claims, and causes of action arising out of or in connection with this Contest, or any prizes awarded, shall be resolved in the following manner: (a) Negotiation - The Parties first shall use reasonable efforts to discuss and negotiate a resolution of the controversy; (b) Arbitration. If the efforts to negotiate a resolution do not succeed, the Parties shall resolve the controversy by final and binding arbitration in accordance with the Rules for Commercial Arbitration of the American Arbitration Association (the “Rules”) and pursuant to the following additional provisions; (c) Applicable Law - The Parties agree that the internal laws of Oklahoma shall govern the construction of the terms and the application of the provisions of the Contest and that all disputes will be resolved in accordance with Oklahoma law; (2) Selection of Arbitrators - The Parties shall select one arbitrator within ten (10) days after the filing of a demand and submission in accordance with the Rules. If the Parties fail to agree on an arbitrator within that ten (10)-day period or fail to agree to an extension of that period, the arbitration shall take place before an arbitrator selected in accordance the Rules; (3) Location of Arbitration. The arbitration shall take place in Oklahoma City, Oklahoma, and the arbitrator shall issue any award at the place of arbitration. The arbitrator may conduct hearings and meetings at any other place agreeable to the Parties or, upon the motion of a party, determined by the arbitrator as necessary to obtain significant testimony or evidence; (4) Enforcement of Award - The prevailing party shall have the right to enter the award of the arbitrator in any court having jurisdiction over one or more of the Parties or their assets. The Parties specifically waive any right they may have to apply to any court for relief from any decision of the arbitrator made prior to the award; (5) Attorneys’ Fees and Costs - The prevailing party to the arbitration shall have the right to an award of its reasonable attorneys’ fees, costs, and expenses incurred after the filing of the demand and submission.

15. LIMITATION OF LIABILITY/GOVERNING LAW: All entrants agree that by entering the Contest they waive any and all rights to claim actual, punitive, incidental, and consequential damages, and any other damages, against Sponsors and their designees or agents. All issues and questions concerning the construction, validity, interpretation and enforceability of these official Contest Rules, or the rights and obligations of entrants and the Sponsors and their designees or agents in connection with this Contest shall be governed by, and construed in accordance with the laws of the State of Oklahoma, without giving effect to any choice of law or conflict of law rules or provisions (whether of the State of Oklahoma or any other jurisdiction), that would cause the application of the laws of any jurisdiction other than those of the State of Oklahoma. WITHOUT LIMITING THE FOREGOING, EVERYTHING REGARDING THIS CONTEST, INCLUDING THE PRIZES, IS PROVIDED “AS IS” WITHOUT WARRANTY OF ANY KIND, EITHER EXPRESS OR IMPLIED, INCLUDING BUT NOT LIMITED TO, THE IMPLIED WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE OR NON-INFRINGEMENT.

16. RIGHT TO USE NAME, PHOTO, VOICE, AND LIKENESS: By participating, all participants automatically grant Contest Facilitators, Griffin Media and/or TriadEye Institute, and their designees the right to use his/her name, voice, photograph, image, likeness, address (city and state), biographical information, prize information and/or statements about this promotion for advertising and publicity purposes in all media (including social media) at the discretion of the Contest Facilitator(s), without limitation, for an unlimited time and without further compensation or notice.

17. NO AFFILIATION: This promotion is in no way sponsored, endorsed, or administered by, or associated with Facebook, MailChimp, Meta, or Instagram.
